Low Stomach Acid and Celiac Disease

I had written a smaller shorter post previously that seem to resonate with people and am now including updated post(s) that include more of the things I have learned since then.

Dear Gluten Intolerant,

Low stomach acid has now been linked to a probable cause of damage to the Small Intestine before and/or occurring with a Non-Celiac Gluten Sensitivity (NCGS) or Celiac diagnosis.

See this research as reported on celiac.com that discusses the increased risk of/for someone to develop celiac disease after taking PPI’s.

Note how the article starts quoting

“Rates of celiac disease and the use of drugs to inhibit the secretion of stomach acid have both increased in recent decades. A research team recently set out to explore the association between anti-secretory medication exposure and subsequent development of celiac disease.”

If these medicine are lowering stomach acid what cause effect relationship does/ could this have on Celaic/NCGS diagnosis is what he is postulating.

He goes on to say without being very technical (read the whole article for yourself) that

“The data clearly show that patients who use anti-secretory medications are at much greater risk for developing celiac disease following the use of these medicines.

The fact that this connection persisted even after the team excluded prescriptions for anti-secretory medicines in the year preceding the celiac disease diagnosis suggests a causal relationship”.

If even after a year OFF these medicines your chances of developing Celiac Disease (CD) not to mention even NCGS which is much more prominent surely the researcher is correct in postulating that there is a cause and effect relationship between low stomach acid and NCGS and/or Celiac disease.

Surely there is something we can learn here.

I now postulate some homework for the reader of this blog post.

Do some research for yourself and see if achlorhydia or hypochlorhydia symptoms don’t at least resemble in some manner all of the GI symptoms you have been having. (I note some of the many symptom’s low stomach acid can present with below as referenced from Dr. Myatt’s online article “What’s Burning You” for easy reference (It might not be what you think (my words))

It is important to note here that “some” symptoms does not mean all but many or several.

It is called a differential diagnosis.

It is an important diagnostic tool in medicine. Think of the tv show “House” where they spend the whole hour/over a week times going through the ‘differential diagnosis’ in short any one symptom can/have many different causes.

The trick is how to quickly eliminate possible outcomes as symptoms (many) go up.

All is usually never meet because that would make the disease in full outbreak and obvious even to the layman a condition described as “frank” or “classic” Scurvy or Rickets as an example.

Sadly too often after 8 to 10+ years of testing after all the differential diagnosis’s are ruled out you are said by process of elimination to have Celiac Disease if you are lucky or maybe NCGS and not some other acronym GI disease as I like to refer to them as a group.

GERD,IBS,UC, Chrons etc because if they turned down that street … . you are/could be in/at a dead end for they stop looking at the trigger (gluten) as the cause of your gastric upset/digestive disorder(s).

So in summary if 3 or 4 or 5 or 6 of these symptoms overlap “many of’ these symptoms could be Low Stomach Acid related.

IF that is the cause/case for you then there is hope! For remission!

When low in stomach acid we become low in essential nutrients

Quoting from Dr. Myatts “what’s burning you” article

“Our bodies need 60 or so essential nutrients. “Essential” means that the body MUST have this nutrient or death will eventually ensue, and the nutrient must be obtained from diet because the body cannot manufacture it. Many of these essential nutrients require stomach acid for their assimilation. When stomach acid production declines, nutrient deficiencies begin.

Calcium, for example, requires vigorous stomach acid in order to be assimilated. Interestingly, the rate of hip replacement surgery is much higher in people who routinely use antacids and acid-blocking drugs. We know that people who have “acid stomach” were already having trouble assimilating calcium from food and nutritional supplements due to lack of normal stomach acid production. When these symptoms are “band-aided” with drugs which decrease stomach acid even more, calcium assimilation can come to a near-halt. The result? Weak bones, hip fractures and joint complaints resulting in major surgery.

Jonathan Wright, M.D., well-known and respected holistic physician, states that “Although research in this area is entirely inadequate, its been my clinical observation that calcium, magnesium, iron, zinc, copper, chromium, selenium, manganese, vanadium, molybdenum, cobalt, and many other micro-trace elements are not nearly as well-absorbed in those with poor stomach acid as they are in those whose acid levels are normal. When we test plasma amino acid levels for those with poor stomach function, we frequently find lower than usual levels of one or more of the eight essential amino acids: isoleucine, leucine, lysine, methionine, phenylalanine, threonine, tryptophan, and valine. Often there are functional insufficiencies of folic acid and/or vitamin B12.”

Remember, these are essential nutrients. Deficiencies of any single one of them can cause serious health problems over time. Weak bones, diminish immune function, failing memory, loss of eyesight and many other “diseases of aging” are often the result of decreased stomach function.”

It is me again reader.

So low stomach acid is the triggering agent (often) for low nutrients.

Make too much sense!

Now don’t take Vitamin’s for this condition where low Vitamins/Minerals are known to be low in patients who have this condition because … . (if you do you won’t need to keep coming back to the doctor) I can almost hear the doctor say now.

Of course he/she doesn’t say that … just that the “average person” doesn’t need to take Vitamins.

Well I hate to break it too you … . but if you are having GI problems and reading this blog post on the celiac posterboy’s blog then you are not the “average” person.  You my friend or a sufferer or a friend of a sufferer still looking for answers.

If that is you then consider taking either powdered stomach acid – Betaine HCL or taking Niacinamide to help you reset your stress clock.

A Canadian researcher wrote about this connection 15+ years ago but still most doctors’ don’t understand this connection between about how “Niacin treats digestive Problems”

I didn’t believe it myself for over a year .  . . but every time I thought about it – it (Low Stomach Acid) made/makes the most sense to me.

Don’t take my blog post as medical advice.  It is only what I did … it might not work for you but I think it is worth a try especially if you are not now taking an acid reducer. (see notes below about why this might) be more difficult if you are already taking an acid reducer … because the rebound wall (see/google chris kresser  what everybody should know about stomach but doesn’t) keeps us locked in … sometimes for years.

Since I was not taking acid reducers at the time I took Betaine HCL my stomach problems improved and I am sharing this now in the hopes it might help yours too!

Now back to (really) LOW stomach acid being diagnosed as HIGH stomach acid these days.

How can we know if it truly high or low?  You’ve heard the phrase timing is everything well it is here too!

Timeline is important in any diagnosis.

IF your stomach acid was HIGH as you often hear (everywhere) you hear take a Proton Pump Inhibitor aka acid reducer’s for heartburn/GERD (medical name for heartburn) then eating food (carbs, greasy things) wouldn’t bother you.

The acid would cut it up but if it is already low/weak then even a little acid can burn your esophagus which is not coated like the stomach to protect you from high acid.

BUT if it is low to start with then food will WEAKEN our/your acid so that you lose the food fight your in and things (carbs/fats) become to ferment, rancidify and cause heart burn.

Leading in time to Non-Celiac disease first and with enough injury (and time) to Marsh lesions qualifying you for diagnosis as a Celiac candidate/ patient.  See above link between/about PPI’s in the year preceding a Celiac diagnosis.

If you (can) be that patient and weight the xx number of years for all this damage to occur, there is a better way it is called digestion!

A virtuous cycle can replace the vicious cycle you are now in – it is caused digestion.

Digest your food with healthy stomach acid and your body will thank you for it with the God given burp.

A healthy child burps (at 6 months of age normally) and a healthy adult should too and you will again after taken Niacinamide 3/day for 6 months or this is not the right diagnosis.

If you are a) experiencing heartburn that causing vomiting (with unintended weight loss) you may have a special case of heartburn that feels like heartburn (on steroids) that is really Bile Reflux and taking Stomach is not something you should do without medical advice and supervision.

b) you are already taking an acid reducer then the chance you will get better (off of acid reducers completely) is only 50/50 on your first try but going low CARB can help your transition.

Otherwise most people will get better when taking BetaineHCL for gastric support and Niacinamide to help them/you reset your digestive processes.

Taking it (Niacinamide) (or any B-Vitamin) should be taken 2 to 3/day (too keep up serum levels) for 3 to 4 months (the time you can store B-Vitamins) in the liver mostly.

Once you have a distinctive BURP that displaces the bloating and sense of “I am going to explode” if I eat another bite (though you haven’t eaten half your meal) then normal digestion is occurring again.

If you stool did not sink before this process (of taking Niacinamide begun) and burping became your “new normal” then it (your stool) will begin to sink too!

Burping without bloating is the “Natural Order” of good digestion.

This (low stomach acid) thing is a vicious cycle. STRONG stomach acid makes it a virtuous circle/cycle.

All heartburn is not equal. IF your stomach acid is truly high then it WILL occur between meals when there is no food to tamp down the fire (occurring in your stomach) not your esophagus.

The excess pressure from fermented carbs push open the trap door allowing the low acid you have to burn the lining of your uncoated esophagus.  (More on that later) in the next post.

But I think you will find in 3 to 4 months of taking Niacinamide 3/daily that  as a healthy child burps at 6 months? right that is how we know it’s digestive system is strong enough to digest real food.  That it it’ true for adult’s as well.

After 6 months of no flareups or “accidental” glutening’s even when your friends still suffer from “good” compliance/control from background gluten or gluten cross contamination issues  18 to 20 hours after the “glutening” you will be once a day regular.

This research is 15+ years old and doctors have not caught up to this fact either.  You needn’t suffer any longer if your LOW stomach acid is being misdiagnosed as NCGS, GERD, Heartburn, IBS, and yes even Celiac Disease then remission from LOW stomach is possible.

A diagnosis of Pellagra will likely have many other disease presentations, not limited to acne, rosacea (dermatitis) skin rashes, depression, anxiety, dementia, etc., as well. Seventy-five or more years ago the symptoms of Pellagra were commonly diagnosed as separate diseases that were known to be of a common cause. Similarly celiac disease today is often referred to as the “tip of the iceberg,” not only because of the large number of undiagnosed people, but also due to the vast range of symptoms and associated diseases and disorders that can be caused by untreated celiac disease. I assert that Pellagra could be, in some cases, related to Celiac disease, and might even cause it.

Today Pellagra has mainly been reserved for subgroups of people like alcoholics or disadvantaged groups like the homeless, who lack good protein sources in their diet to stave it off. Quoting Niki Frost from the Pellagra Disease Blog: “Since pellagra is such a rare disease today, few people recognize it based on their symptoms alone (any of the three D’s: dementia, dermatitis, and diarrhea). Pellagra patients are consequently referred to specialists in fields that are ultimately unrelated to the underlying nutritional deficiency and metabolic nature of the disease.” (1)

Dr. Heaney, Professor of Medicine at Creighton University, actually echo this fact when he says “In the United States, at least, Pellagra is a disease of the past, fortunately, and it is doubtful today that most health professionals would recognize it if a case happened to come to their attention.” (2) Why is this? Because, as mentioned above, doctor’s don’t recognized it today unless they are a specialist, and even then, according to Nicki Frost: “specialists (are) in fields that are ultimately unrelated to the underlying nutritional deficiency and metabolic nature of the disease.” (3) So the cycle repeats.

Since these specialists might not consider people with celiac disease/Non-Celiac Gluten Sensitivity (NCGS) to be in a subgroup which is prone Pellagra, is it possible that some people are misdiagnosed? Skin conditions like acne, dermatitis, eczema, psoriasis, skin lesions, and mouth ulcers can be related to both Pellagra and celiac disease or NCGS. Some of the other signs of Pellagra, other than digestive distress, are dementia related symptoms such as depression, anxiety, brain fog, mental confusion, etc., which are also present in 40 percent of celiac patients. What if doctors haven’t made the right diagnosis in some cases? Many people who might be misdiagnosed with celiac disease but who actually have Pellagra might not get better because they are not treating the right disease.

Pellagra and its associated skin and GI problems are totally reversible with supplementation, which is one of the reasons that many foods are enriched with niacin. Glutenfree Works covers this topic well by noting a casual relationship, but not a causal relationship. They even note that a niacin deficiency itself can cause some of these symptoms. According to Glutenfree Works:

“Intriguing animal research by Sandhur et. al. has shown that niacin deficiency itself sensitizes the intestinal mucosa of rats to gluten in wheat, barley, rye, oats and corn and induces susceptibility to gluten toxicity by means of cellular dysfunction. Human research needs to investigate this effect of niacin deficiency in human celiac disease.” (4)

But no one else seems to take note of this amazing fact. The research by Sandhur et. al. is 30+ years old, but is it possible that people are suffering from all kinds of GI problems, possibly even celiac disease, because they are ignorant of this fact?

Around 15 years ago Prousky did follow up human research (5) that supports the idea that Niacinamide can indeed treat different digestive problems. It was this initial research which I read 10+ years ago that lead me to the hypothesis that Pellagra today is might be misdiagnosed as Celiac disease, because doctors are no longer able to recognize Pellagra in a clinical setting, having not seen it 75+ years.

From Dr. Heaney’s blog: “2014 marks the 100th anniversary of the war on pellagra, a war that lasted nearly 25 of those years before victory could finally be declared. You have not heard of the war on pellagra? The celebration is not on your calendar? You’re not alone. Why did it take so long? Was the science so intractable, like the current “war” on cancer? No. It was politics and pigheadedness that were the obstacles.“ Dr. Heaney says: “Nutritional deficiency was not an accepted category of disease when Goldberger started work” on his cause/cure for Pellagra, and I must also mention that it is not looked at today in those diagnosed with celiac disease. (6)

Let’s look into this more if we can. Let’s see what other expert’s say about this. If we look at back issues of The International Journal of Celiac Disease we see a casual link that indicates a co-morbidity of Pellagra in celiac patients of 58%. (7) Note they the International Journal of Celiac Disease is not looking for a causal link but they only report casual association. In the article entitled “Two Exceptional Complications Revealing Celiac Pellagra” the author notes: “pellagra is essentially affecting tissues with a high rate of cell turnover, such as the digestive tract and the skin, and tissues with high energy needs, such as the brain”. (8) If the symptoms are so similar, it might be easy to confuse one disease for the other.

How is this possible? The International Journal of Celiac Disease muses on this point too. When discussing the “exceptional” and not well understood reasons why Pellagra might show up in a celiac diagnosis, they say “Little progress has been made in our knowledge of pellagra…since Goldberger discovered that nicotinamide was a preventive factor in 1926…(P)roof of this is that there have been no changes in treatment or diagnostic criteria in the last 90 years.” The International Journal of Celiac Disease summarizes this case of “Exceptional complications revealing celiac disease and Pellagra illustrates…rare complication revealing celiac disease”. (8)

Since up to 58 percent of celiac patients might be co-morbid with Pellagra, could it be possible that Pellagra is the “parent disease,” and the those diagnosed with celiac disease have symptoms derived from Pellagra? If we follow most normal paths for adoption it will take another 20 years (a generation) for the medical community to accept Pellagra as perhaps the proper diagnosis in some cases. So there is some evidence to suggest that medical science is, perhaps in many cases, identifying the wrong disease. If one is critically low in niacin, the 3 D’s of Pellagra (Dementias, Dermatitis’s, and Digestive Issues) show up.

Take the “Niacin (Niacinamide) Challenge” and see if taking it three times a day for six months don’t cause a healthy GI pattern, including a once per day bowel movement that sinks and healthy burping without bloating. Taking the niacinamide Challenge may put co-morbid cases of Pellagra, including its digestive symptoms, into remission, and provide relief for many who do not recover on a gluten-free diet alone. The number one mistake people make when taking niacinamide is they don’t take it for long enough. It takes 4 to 6 months to overcome a serious deficiency of this vitamin, and for your mucus membranes (GI lining) to heal itself.

Once you recover you can maintain your health, barring some future stress/trauma that depletes your reserves, at which time heartburn/GERD/IBS/diarrhea/constipation symptoms will return, and you will lose your ability to burp easily again. And the cycle repeats and the Pellagra symptoms come back with a vengeance.

It should be noted that the forum member had already treated her DH with Niacin and already knew of its power to help Dermatitis problems.

Soon after the is article appeared on Celiac.com someone on my Celiac Posterboy blog began taking Niacinamide for her GI problems associated with her Celiac disease that were still having Constipation and Diarrhea problems despite being gluten free and got better (her C&D settled down) shortly after beginning the Niacinamide.

I have Celiac and have been having a terrible time with “D” these past 3 months. Read Celiac. com and your article about the Niacinamide was very interesting. Due to the problems with the GI tract, I was using a BRAT diet more often than I should have. While I do take a multi with niacin in it, probably need a lot more. Added the Niacin and Niacinamide and already things are slowing down. Yea! Just had colonoscopy today and EGD to r/o any problems, Doc said that things looked good but he took biopsies to r/o SBIO and/or another type of colitis. Once I added the niacin things started to improve. I am going to take your article to his office when I do my follow up visit. Thanks for writing it.

This reminds of a quote from a friend of mine J. The Truth that there is  the FATHER who is the ONE God who had one begotten Son Jesus Christ/Messiah our Lord is truly good news for us. Because He (God) has raised that one man from the dead giving us assurance he someday will raise us too if we are in Christ (The Anointed Jesus)

While this issue of error of getting a wrong diagnosis does not raise to this error of being dead forever (the opposite of eternal life) it can lead to an early grave if you continue to have Pellagra and are not treated for it properly – the 4th D of Pellagra.

The knowledge (truth) that a Vitamin can help us is greater than the error of being misdiagnosed with the wrong disease.

If the International Journal of Celiac Disease is correct there is better than a 50/50 chance you will get better the way Knitty Kitty, Mairllyn, and the Celiac Posterboy has.

Because Pellagra is progressive you will die from the 4th D of Pellagra in time.

Someone can be said to “die before their time” don’t let Pellagra steal your time away from you any longer.  You needn’t die before your time if you take the Niacinamide and get better from it.

Taking it and getting better is considered sufficient proof that a Niacin(amide) deficiency was causing the 3 D’s of Pellagra.

A simple urine test is all that is needed to determine if you have Pellagra and the doctors’ have not recognized it in a clinical setting.

They summarize it very well why Pellagra is not being diagnosed today in Doctor’s offices’ across the land.

Pellagra in Italian (the language it was first described in 250+ years ago) meaning “Rough/Sour Skin”

In their diagnosis section quoting  “The diagnosis of pellagra is straightforward when the classic rash is present but may be elusive if there are only gastrointestinal and/or neurological manifestations.”

Today if you fail to get the “classic symmetrical” sun (photosensitive) rash then you fail to get diagnosed as a Pellagrin in the first place then the other Ds’ of Pellagra present such as the Gastrointestinal issues associated with a Celiac diagnois and/or neurological problems like the 40% of those who are get a Celiac diagnosis and are depressed but are diagnosed with it as part of a separate diagnosis from  that of a Celiac diagnosis.

Or if you quoting again don’t have what they (according medical doctor’s diagnosing guidelines)  consider  “Poor dietary habits, such as those found among chronic alcoholics, individuals with severe eating disorders (e.g., anorexia nervosa and bulimia nervosa), and the malnourished also increase the risk of pellagra” then you are not diagnosed today as having Pellagra.  Today you will be diagnosed as a Celiac.

Since you are not one who consumes/relies on a quoting again “a diet high in corn products” then you the wheat consuming Pellagrin are diagnosed as having Celiac today instead.

Oh you cope, manage, continue to have flares get more sensitive to other proteins lactose, soy, corn (there is that corn thing again) etc.

You have removed gluten from your diet… you should get better.

The Celiac Posterboy who writes/blogs about the digestive disorders that Pellagra causes today often presenting as other diseases such as GERD, IBS, UC, Chrons or other GI problems and even NCGS and in Time Celiac Disease.

And now I want to pivot and include other disease presentations so called extraintestinal (out side the GI) as the term is used that occur in other parts of the body.

Too often the 20+ associated diseases of Celiac disease have other non GI related disorders commonly diagnosed as separate diseases of a common origin.

Quoting the International Journal of Celiac Disease (from here on out IJCD) article Celiac Disease: Intestinal, Heart and Skin Interconnections http://pubs.sciepub.com/ijcd/3/1/6/

“It has been shown that the classic intestinal clinical picture of malnutrition, chronic diarrhea and nutritional deficiencies are disappearing and extraintestinal presentations are emerging. Skin, cardiological, endocrine, skeletal, hepatic, hematological, thrombophilic, gynecological, fertility, dental and behavioral abnormalities are often described [1, 2, 3, 4, 5].” And I would add dementia related conditions should be included in the differential diagnosis process.

Quoting the IJCD again “All these extraintestinal presentations make the diagnosis of the disease more difficult and the reliance on the typical symptomatology more remote. These are some of the reasons why serological screening and diagnosis of CD have achieved prime importance and the high-risk CD populations were expanded to other conditions and to extraintestinal target(s) organs symptomatologies [6, 7].”

And should be expanded to the brain and its many symptomtaologies (their word) like the 3rd D of Pellagra: Digestive, Dermatitis and DEMENTIA’s like anxiety, depression etc.

It is commonly known as gluten ataxia if associated with a Celiac diagnosis today.

But it should be noted that gluten ataxia’s have shown up in other dementia related disorders.

But is it really Pellagra in disguise today.

The problem today is Pellagra is no longer being diagnosed as the cause of these Dementia’s.

Hindwai (a medical reference website notes this) “subpellagrous nicotinamide deficiency may have lifetime roles in a range of behavioral traits, neuropsychiatric diseases, and dementias.” https://www.hindawi.com/journals/cggr/2012/302875/

They (Hindawi) say concerning Neurological presentations of Pellagra “Famously, pellagra caused dementia(S), a characteristic photosensitive dermatitis (Casal’s necklace), diarrhea, and death. (Is my word(s) because you will see below it is not just a dementia but many dementia syndrome/diseases today that were once linked to Pellagra 75+ years ago today.

Quoting the Hindawi Journal again

“Mental symptoms were wider than dementia, in that depression, fatigue, psychomotor retardation, mania, obsessions, and a whole range of psychoses with auditory and visual hallucinations were well described, along with personality change and sociopathic and drug and alcohol addictive behaviours. Panic disorders were seen as was a general inability to deal with physical or mental stress. Poor brain development such as hydrocephalus or cerebral palsy was also common. Acute delirium or even coma occurred, with some patients having myoclonus and other extrapyramidal signs reminiscent of the spongiform encephalopathies. The dementias (their words) of pellagra included features akin to Lewy body, Alzheimer’s, frontotemporal, vascular, and prion diseases. Parkinsonism was also common and a festinant gait was first described in pellagrins. (see above how the gluten ataxia’s of a Celiac diagnosis now mimic ALS aka Parkinson disease (today)! Tremors of various descriptions, including asymmetric rest tremors, were noted and some patients had typical paralysis agitans. Pellagrins had a characteristic expressionless facies, so some signs of parkinsonism were present in most cases. Many features of pellagra closely resemble the nonmotor aspects of PD.

The neurological manifestation did not stop there because other degenerative conditions, such as an amyotrophic lateral sclerosis-like picture, were described, with fasciculation of the tongue and upper and lower motor neuron signs. Cerebellar syndromes occurred and vertigo was frequent. Headaches, sensory and pain syndromes, epilepsy, and involuntarymovements were noted as well as sleep disturbances. Cord lesions were also seen, as was optic atrophy, so there were multiple sclerosis (MS), like variants.”

This standard research article/work from Hindawi is meant to serve as “Lesson’s Learned” White paper of sorts and they even subtitle it such “Hypthoses and Lessons from Pellagra” the problem is they are not Lesson’s Learned TODAY.

Dr. Heaney notes the same thing when talking about the 100 year anniversary of the War on Pellagra being over . . . . but the battle rages on. When he says . . . “at least, pellagra is a disease of the past – fortunately – and it is doubtful today that most health professionals would recognize it if a case happened to come to their attention.” Amen to that!

Susan is much kinder than Dr. Mercola who she references in her blog where Dr. Mercola notes violent tendency in patients/inmates in psyche wards and prisons who when given Niacin went into remission at a rate as high as 50 percent with low grades supplementation and at rates as high as 80 percent with high dose supplementation when Dr. Hoffer in the 1950’s first discovered Niacin treats the Dementia’s of Pellagra.

And it was not just mild mental symptomtaologies like being sad that were cured either but even schizophrenia.

Dr. Mercola asks?

What about the people that had a little bit of niacin, but didn’t get better? … [H]e Dr. Hoffer started giving what at the time were preposterously high doses of niacin: 3,000 milligrams a day. And he was curing schizophrenia in 80 percent of the cases

(It should be noted for clarity the 3,000mg was in divided doses or multiple times a day.)

This explains well the violent tendency of advanced Parkinson disease patients.

A noted characteristic of the disease advancing to its end state: death the 4th D of Pellagra.

More on that in another post but here is research that even confirms this hypothesis making it no longer a hypothesis but a “working” theory.

Note this information is 10+ years old and still people are searching for this “magic” cure in the way of a medicine/drug that “cures” what a Vitamin has proven to prevent.

They did not say casual link (occurs with) but that Niacin is a CAUSAL dementia of Pellagra (occurs because of) a Niacin(amide) deficiency now being diagnosed TODAY AS Parkinson’s disease.

Yet people are still looking for a cure somewhere else.

Now again noting the Dementia’s of Pellagra article Dr. Mercola notes other researchers have confirmed Dr. Hoffers’ earlier findings as covered on Susan Huges healthy by Nature blog link provided again for easy reference http://blog.healthybynaturehwc.com/niacin-deficiency/

Dr. Mercola Quoting “According to Dr. Saul, other researchers have since confirmed Dr. Hoffer’s findings, and found that niacin can also be successfully used in the treatment of other mental disorders, such as:

* Obsessive-compulsive disorder

* Bipolar disorder

Pellagra is an extreme or total niacin deficiency and Pellagrins have GI problems and many other things – including having mental illness symptoms.

That Doctor’s 75 years ago diagnosed as Pellagra but since our bread has been enriched with Niacin depressed people no long receive this diagnosis.

Young men from affected states had inordinate difficulty in passing the intellectual tests that involved reading, writing and numeracy skills and the physical fitness tests required by the military, suggesting that subclinical disease was rife.”

It just goes by other names today instead of Pellagra. Mostly Celiac disease. 58% of Pellagrins today are diagnosed as having Celiac disease according to the International Journal of Celiac Disease (IJCD).

Try it for yourself and see.

Take the Niacinamide Challenge taking Niacinamide 500 mg 3/day for 6 months and see if you don’t begin burping in 3 to 4 months and your moods get better in the first 6 months of taking it.

SADLY few do get a correct diagnosis. “Snap out of it” we are told. When all along our bodies are not able to function properly. When that function is restored with something that I like to call some TNT (Targeted Nutrition Therapy) then many illness can be blasted away.

The key here this is a cycle of supplementation. You won’t always need to take Niacinamide FOREVER like a pill (a pill for everything) common in medical practice today.

Now the cycle might be a year or two depending on how often you take and how low you are in it but

It is a cycle and then you can cycle off it the way I have.

Though most people will benefit from a cycle of taking it (Niacinamide) 2 or 3/day for a year if you are low enough to have DEMENTIA’s of Pellagra.

But as long as you give yourself two hours between dosages then you can take another Vitamin because this is a water soluble Vitamin your body will not store more than it needs at that particular moment.

It will take 6 months of 3/day before your body refills the small reserves it is able to store in the Liver mostly.

The difference in fat soluble Vitamins (A,E,D,K) can be stored for years in your fat cells.

After your limited storage site is full your body will now be able to restore normal functioning and many of the D’s of Pellagra will improve like Dermatitis issue’s (roseaca, eczema, acne, psoriasis etc) and Digestive Disorder’s like GERD, IBS, Constipation, Diarrhea etc.and your moods (Dementias of Pellagra) will improve dramatically.

In case this is the only post you read I will repeat here for knowledge sakes ****** the number one mistake people make when taking Niacinamide is they don’t take it long enough or frequent enough to make a difference. It should as I have said elsewhere in this blog it should be taken UNTIL you are burping and I might add burping without bloating and your stool now SINKS where it did not before.  A sign that the Vitamin is working to restore healthy function again after many years of being chronically low in Niacin(amide).

Most people will burp when taking Niacinamide in 4 to 5 months or less taking it 3/daily — ie with each meal. They will become once a day regular if you are not already (for those who are already gluten free) and your stool will sink to the bottom of the bowl a sign of completed digestion. Usually taking a B-Vitamin only once a day will not make a noticeable difference in your symptom’s because after 2 hours your body is low again. B-Vitamins should be taken multiple times a day for best effect. At least 2 to 3 times daily.

The smaller the dosage 50 to 100mg of Niacin (and the less issues of flushing with the Niacin form) the more often you can take it. Smaller more frequent (3 to 4 times daily) dosages work 40% better than larger (500mg (1 to 2/times a day)) less frequent doses.

It should be noted also that at 500mg doses of Niacin will cause flushing. So it is better to find the non-flushing Niacinamide form for best results. Niacin flushing can mitigated by eating it with food but expect a substantial flushing when taking Niacin in doses above 100 mg. Niacin usually only flushes for 2 or 3 days before depleting your histamine reserves over that time.

How Acne, Rosacea, Psoriasis, Eczema, Hives and other Dermatitis issues like DH commonly present in Celiac/or other GI patients is actually one of the 3 faces of Pellagra by the Celiac Posterboy

I never know where my blog post will lead me.  And this deserves its own separate blog and not just one post and who knows there might be more posts on this subject but I will start with one.

Now I pivot in my blog to talk about the Dermatitis’s of Pellagra.

And why I do not know the number (percentage) of Skin problems that occurs in Celiac’s or other GI problems I will for easy math assign them one third (1/3) each for each D of Pellagra. Though I think it has as much to do with the depth of the deficiency since all 3 D’s will present themselves in advanced Pellagra cases.

Just Google some images of Pellagra patients and almost all them will show with “rough/sour” skin conditions (see below for how Pellagra originally got its name)

Why more than one condition often exist in many GI patients like depression and dermatitis issues.

The doctor’s don’t often understand they have common origins today.

I will summarize quickly if this is the only blog post you read of mine.

And brings me to the reason for my post today, I see this a lot and thought a post on why dermatitis issues presenting with GI problems are related and why doctor’s often don’t recognize these connections today was needed.

“You in the back of the class, yeah you reading this blog” Do any of you know what Pellagra means?

Probably not, I guess unless you know a little Italian or Latin.   When Pellagra first came on the scene as a diagnosable disease medicine was really just getting its start as science.

Pellagra was first diagnosed as a SKIN disease, Class.  I bet none of you knew that – that your “rough/sour skin” was diagnosed as a disease 250+ years ago and cured by a Vitamin 75+years ago yet still doctors’ today struggle with diagnosing it today as a Niacin deficiency.

Continuing the Wikipedia citation “ Pellagra was studied mostly in Europe until the late 19th century when it became an epidemic especially in the southern United States.[29] In the early 1900s, pellagra reached epidemic proportions in the American South. Between 1906 and 1940 more than 3 million Americans were affected by pellagra with more than 100,000 deaths, yet the epidemic resolved itself right after dietary niacin fortification.[30]”

The point is not to make you Pellagra experts here but to educate.

Let’s look at the last sentence in this wikipedia citation “Between 1906 and 1940 more than 3 million Americans were affected by Pellagra with more than 100,000 deaths, yet the epidemic resolved itself right after dietary niacin fortification.”

YET the ( Rosacea, Psoriasis, Eczema my words) ie, Pellagra epidemic resolved itself right after dietary niacin fortification.

And you wonder how they could ever forget?  That Pellagra means rough skin.

Did you now a Vitamin could help your Acne?  Well it can the way it did 75+ years ago when it first discovered.

It is often one of the early signs of someone with undiagnosed Pellagra today.  I see it a lot and why I am writing about it today.  I give them my card “How to Fight Gluten/GERD and Win or How the Doctor’s Almost Killed Me” but people never make the connection.

They look at me with a blank stare on their face not realizing that even something as simple as a Vitamin can help their inflamed skin.

Yes Acne is inflammation and Vitamin B-3 in the Niacinamide form when put into someone’s face cream often helps people with Acne.  At my local vitamin store Niacinamide is on standing orders from the local Dr.’s of Dermatitis because they know it helps 10 to 15 percent better than prescription medicine.

They don’t recognize it as Pellagra mind YOU they just know it helps.

Why is this possible?  What is the connection and how can one Vitamin be responsible for so much misery and suffering? When we are dependent on it? It begins to make a lot more sense.

Well we all need energy to survive no doubt we would agree.  Niacinamide plays a special role in the Mitochondria of every Cell in our body to make energy.

This is most pronounced in area where there is high cell division.  Think the energy needed for pregnancy/to have a child.  And the cell division that goes on when we are constantly replacing (shedding) old skin and replacing it with new skin and you begin to see how this could affect so many body systems/organs in the body.

Note: This is the same reason pregnant people get Acne and develop heartburn late in the pregnancy.

Yet they don’t know you have become low in a Vitamin due to your high stress (energy) needs.

The same is so for the GI system.  Our body replaces the mucus linings of our stomach approximately once a quarter or every 3 to 4 months.

Heartburn/GERD in pregnant people really is Gestational Pellagra and the Doctor’s haven’t understood the increased need by your bodies for Niacinamide when we are stressed by pregnancy.  And they need more Niacinamide to make the energy necessary to prevent the destruction of the lining of the stomach and skin layers.

The replacement of these cells (epithelial) according to the Oxford University Press definition

“relating to or denoting the thin tissue forming the outer layer of a body’s surface and lining the alimentary canal and other hollow structures: ie “the epithelial cells lining the gut” ·

The stomach can be a harsh place so these cells are constantly replacing themselves as well as the tough outer layer of epidermis which takes the brunt of the damage for your body because they are the first layer of tissues (skin) etc that invaders/bacteria, viruses etc come in contact with.

So without knowing it your body is constantly using a lot of energy to maintain itself.  This constant energy drain on our system can make us low in time (usually 3 to 4 months) the exact time/amount you can store Niacinamide in our liver.

When you get below a critical level you can no longer maintain or repair the lining of the stomach or your skin and it breaks down and you develop acne, psoriasis and other dermatitis issues etc.

So if you want to get rid of your sour/rough skin ie Pellagra then topically apply some Niacinamide to work from the outside in (hydrating) and the inside out (rebuilding) your epithelial layers (skin and GI linings) so it can continue to protect you.

In case this is the only post you read I will repeat here for knowledge sakes ****** the number one mistake people make when taking Niacinamide is they don’t take it long enough.  It should as I have said elsewhere in this blog it should be taken UNTIL you are burping and I might add burping without bloating and your stool begins to SINK where it probably floats now.  Most people will burp when taking Niacinamide in 4 to 5 months or less taking it 3/daily — ie with each meal.

It takes 4 to 6 months to overcome your dependency on this Vitamin for your epithelial layers (GI lining/tissue/Skin/epidermis) to heal itself. And maybe longer depending on how deficient you are but these should be expected outcomes for most people who try both a topical treatment of Niacinamide in their skin lotions and oral in gestation by pill/capsule.

What is the sign that your body is healing from the inside out.  From the outside in you will see improved dermatitis conditions like better acne control probably in the first few months.  From the inside out once your GI is healed itself because it has enough Niacinamide to repair itself you will begin BURPING for the first time in years if ever depending on how long you have had your deficiency.

This should be an/your “Aha” moment when your skin flare ups begin to go into remission because if it is able to repair the Mucus lining of the stomach it is able to do the same to the epithelial cell/layers of your epidermis/skin/acne/Rosacea, Psoriasis etc.

I could write more but this is enough to give you the tools needed to help yourself.  You now know enough to change.  I don’t know if you will only you can decide that for yourself but I routinely help my friends who will listen with Acne usually when they are stressed who no longer need expensive medicine’s that they need to continue to take year after year.

Amazed that something soooo simple as a Vitamin has helped them control their Acne/ inflammation that always seems to “break out” at the worst times.  When they are stressed out their Acne always seems to get worse (or have eaten a lot of gluten triggering an auto immune reaction in their body triggering inflammation).

Co-Morbidity is difficult to recognize but not hard to prove.   You just have to take Niacinamide 500 mg 2 or 3 times a day/daily  for 6 months or so to see if there is a cause and effect i.e., it induces burping where it did not occur before without bloating and your stool SINKS where it floated before.  A cause and affect relationship.

The website Gluten free works recognizes a Niacin deficiency in Celiac’s but like identical twins has trouble diagnosing the correct twin.

This article is 5+ years old but they argue for a Niacin deficiency in Celiac’s and not the Co-Morbid “evil twin” of Pellagra in the same person.

Niacin Vitamin B3 deficiency in celiac disease gluten free works

“Niacin, also called vitamin B3, is required by all the cells of our body making it essential for vitality and life itself.

Niacin is essential for keeping our skin and digestive tract healthy, our brain and nervous system  functioning normally, certain key cell processes repaired, our adrenal glands producing steroid hormones at demand levels, sex glands producing the hormones estrogen, progesterone and testosterone and, most especially, for producing energy to keep our body alive.1

When absorbed from the small intestinal tract, niacin becomes part of a process including more than 200 enzymes involved in metabolism of carbohydrates, proteins and fatty acids, that is, chemical reactions that maintain life.1 Niacin is stored by the liver.2

Niacin must be digested to release its absorbable forms, nicotinamide and nicotinic acid. These molecules are absorbed across the intestinal lining at low concentrations by sodium-dependent facilitated diffusion, meaning they need help to get into the bloodstream.1 Niacin is absorbed at higher concentrations by passive diffusion, meaning the niacin molecules simply cross through the lining.2 That said, absorption is hampered by the swelling and thickening of the intestinal lining that occurs in celiac disease.  Removing gluten reduces the inflammation and allows the lining to heal, facilitating passage of niacin through the lining into the bloodstream.

Intriguing animal research by Sandhur et. al. has shown that niacin deficiency itself sensitizes the intestinal mucosa of rats to gluten in wheat, barley, rye, oats and corn and induces susceptibility to gluten toxicity by means of cellular dysfunction.3 Human research needs to investigate this effect of niacin deficiency in human celiac disease.

Niacin Deficiency

Niacin deficiency due to malabsorption is common in celiac disease before gluten is removed from the diet.1 Niacin deficit can be aggravated by a lack of niacin, or the amino acid tryptophan, in the diet. Our body uses tryptophan to make niacin as a back up mechanism.  Smoking also impairs absorption of niacin.2

Lack of niacin in the diet comes from choosing foods that are low in niacin. An example of how this can happen is if we used plain corn flour rather than masa flour for baking gluten-free recipes and purchased tortilla chips, tacos and snack foods made from plain corn flour.  Plain corn flour is low in niacin and its niacin cannot be digested. Whereas, the niacin in masa flour is made digestible.  Masa is the result of soaking corn in lime, a strong alkali that releases the bound niacin so that it can be digested.2 Lime would be listed as an ingredient on the product’s nutrition label.

Lack of tryptophan in the diet is due to choosing foods that lack this amino acid and avoiding those that are rich in tryptophan, such as eggs and meat. Conversion of tryptophan to niacin requires adequate levels of tryptophan and adequate levels of iron, riboflavin, or vitamin B6.2 Tryptophan-to-niacin conversion can easily be subverted if tryptophan itself, or the co-factors iron, riboflavin or vitamin B6, are deficient.

Deficiency Symptoms

Inadequate levels of niacin slows metabolism and impairs functioning of the brain and nervous system, digestion, skin, DNA repair and other key cell processes, and steroid and sex hormone production. Severe deficiency of niacin causes the disease pellagra.2

Mild Niacin Deficiency Symptoms:

1. Problems with metabolism: muscle weakness, lack of energy, weight loss, decreased tolerance to cold.

2. Problems with the nervous system: irritability, dizziness, fatigue, poor concentration, poor memory, anxiety, apathy, depression, headache, neuritis (numbness, burning in various parts), tremors, restlessness and hyperactivity in children.

3. Problems in the digestive tract: anorexia, distorted taste, indigestion, scarlet colored tongue, burning of the mouth, throat, and esophagus, abdominal discomfort and distention, nausea, diarrhea or constipation.

4. Problems with the skin and mucous membranes: sensitivity of skin exposed to sunlight causing a red, itchy, scaly rash that resembles mild sunburn, and inflammation of mucous membranes of vagina and urethra.1

Severe Niacin Deficiency Symptoms (Pellagra):

Severe niacin deficiency is called pellagra. Pellagra is characterized by the classic 4 “Ds”…watery Diarrhea, severe Dermatitis, advancing Dementia and Death.

1. Diarrhea can be bloody and other digestive disturbances worsen with vomiting.

2. Dermatitis develops symmetrically in areas exposed to sunlight with hyperpigmentation (dark color), thickening and cracking of the skin, itchy red wet areas from chafing, and “necklace” sores on the lower neck.2 Subcutaneous fibrosis and scarring within the skin may be seen in late stages. A bright red swollen tongue which in chronic stage is fissured and is similar to raw meat.4 Inflammation of other mucous membranes worsens with increasing deficit.

3. Dementia is due to neuronal loss (death of nerve cells) with development of amnesia, disorientation, confabulation, and delirium.

4. Death ensues eventually if the niacin deficiency is left untreated.2,3 “

It is me again reader.

She gets it well but she doesn’t know what twin it is she is identifying. If she got the wrong one (disease) then the suffering continues’ often for years but if the Co-Morbidity is treated then improvement can be shown in as little as 6 months’ time. . .

Quoting the Celiac Posterboy in another post “Now if the symptoms’ are similar it is easy to confuse one disease for the other and if not well understood they might be considered “exceptional” or, i.e., not well understood.

How do we know this? The Journal of Celiac in another issue entitled Celiac Disease Intestinal Heart and Skin Interconnections http://pubs.sciepub.com/ijcd/3/1/6/ when listing common aspects of both diseases’ says and I quote “The two diseases can be connected in two aspects. 58% of pellagra patients were shown to have malabsorption and many had intestinal pathology on biopsies [36, 37]. Alternatively, Pellagra was described in CD.”

Not only did/does the Journal of Celiac cite common symptom’s 58 percent of the time in Celiac and Pellagra but they also cite that Pellagra has been described in a Celiac Disease diagnosis.

I would argue that it should be the other way around if the majority (58 Percent) of the time Pellagra symptoms are described in a Celiac diagnosis it stands to reason that medical science is identifying the wrong disease.

I ask again “if you have the wrong disease would you get better!”

Where quoting Dr. Heaney he correctly states “it is doubtful today that most health professionals would not recognize it (Pellagra) if a case happened to come to their attention” Today!

In case this is the only post you read I will repeat here for knowledge sakes ****** the number one mistake people make when taking Niacinamide is they don’t take it long enough.  It should as I have said elsewhere in this blog it should be taken UNTIL you are burping and I might add burping without bloating.  Most people will burp when taking Niacinamide in 5 to 6 months or less of taking it (Niacinamide) 3/daily — ie with each meal.

It takes 5 to 6 months to overcome your dependency on this Vitamin for your mucus membranes (GI lining) to heal itself.

Once repaired you can maintain them barring some future stress/trauma depleting your reserves at which time Heartburn/GERD/IBS symptoms will return and you will lose your burp again.

And the cycle repeats and the Pellagra symptoms come back with a vengeance.

Cleo Libonati, RN, BSN,  CEO and co-Founder of Gluten Free Works, Inc  says  “Severe niacin deficiency is called Pellagra” but Mild Niacin deficiency is Pellagra too! Even when it being misdiagnosed as Celiac disease …. and the symptom’s continue to worsen until Pellagra is now being  described but not recognized  in the MAJORITY of Celiac patients today without the doctor’s recognizing the same symptom’s.

It is called a Differential Diagnosis and since nothing is differentiating them TODAY they are being diagnosed interchangeably but the suffering is the same UNTIL or UNLESS you treat the Co-Morbidity.

In fact the International Journal of Celiac Disease http://pubs.sciepub.com/ijcd/3/1/6/   (I would say Pellagra) notes this fact when they say “The two diseases (Pellagra and Celiac Disease) can be connected in two aspects. 58% of pellagra patients were shown to have malabsorption and many had intestinal pathology on biopsies [36, 37]. Alternatively, Pellagra was described in CD [38 “

According to Dr. Heaney this explains Celiac Disease today very well where he notes Pellagra was characterized by the “four Ds” – dermatitis, diarrhea, dementia, and death. It is a perfect example of a point made in earlier posts in this blog that “most tissues need most nutrients, and that a deficiency of virtually any nutrient impairs virtually every function of the body. This is in contrast to the popular belief that one nutrient may be good for the skin, another for memory, another for the eyes, another for the immune system – on and on – which is simply not accurate.”

The International Journal of Celiac Disease ie Pellagra misdiagnosed notes the same thing.

“It has been shown that the classic intestinal clinical picture of malnutrition, chronic diarrhea and nutritional deficiencies are disappearing and extraintestinal presentations are emerging. Skin, cardiological, endocrine, skeletal, hepatic, hematological, thrombophilic, gynecological, fertility, dental and behavioral abnormalities are often described [1, 2, 3, 4, 5].” Today in Celiac’s.

IJCD Concluding

Thus, because of their deficiency, pellagra is essentially affecting tissues with a high rate of cell turnover, such as the digestive tract and the skin, and tissues with high energy needs, such as the brain [9, 10, 11].  Which incidently is the same place Celiac disease now shows up in patients diagnosis as they call it in “extraintestinal presentations” because these cells need Niacinamide to make the energy to divide.

We can’t repair our mucus lining with enough Niacinamide.

According to the Orthomolecular website http://www.orthomolecular.com/?ctr=supplement&act=show&id=15   “Skin develops a cracked scaly surface, the brain does not function properly leading to confusion and dementia and diarrhea results from the impaired manufacture of the mucus lining of the gastrointestinal tract”  Agreeing with sandhur etal.

Quoting the Celiac Posterboy blog   “Intriguing animal research by Sandhur et. al. has shown that niacin deficiency itself sensitizes the intestinal mucosa of rats to gluten in wheat, barley, rye, oats and corn and induces susceptibility to gluten toxicity by means of cellular dysfunction.3 Human research needs to investigate this effect of niacin deficiency in human celiac disease.”

And this cellular dysfunction leads to “leaky gut” syndrome because the gluten no longer stays in the intestine but wreaks havoc wherever and whatever organ the protein lodges.  This agrees with new research by Columbia University Medical Center that says “these individuals have a weakened intestinal barrier, which leads to a body-wide inflammatory immune response.”

Otherwise you have NCGS or what they term Non-Celic Wheat Sensitivity (NCWS).

But this intestinal tight junction is closed in those people who have enough Niacin(amide) to produce enough stomach acid and repair their mucus lining  thus defusing gluten before it has had a chance to destroy the small intestine and in turn other body organs it lodges in.

I could go on but this is a lot to digest in one post.  It can be confusing (and angering) to be told you have one disease for 20 years and then find out it might be an easily curable (or greatly controlled) disease with only a Vitamin (albeit in what is termed Megadoses by the medical community today) but really is a only Vitamin dependency.

Your body depends on Niacinamide to repair itself’ and when it is not available in sufficient quantities it lets your mucus lining break down leading to autoimmune reactions to common food proteins like gluten and lactose among others.  Note the 8 most common food allergens are all proteins which the body has to fight off or else it could mistakenly let in more harmful proteins like Viruses so it has no choice but to fight off ALL proteins or else it knows bad things will happen and does even if/when it is only a food protein it still attacks like it was a virus.

But when strong stomach acid is present the gluten is fully hydrolyzed into harmless peptides.

A differential diagnosis is one of  the best standard of medicine rarely practiced today and how specialists decide between competing diseases like UC or Chron’s or IBS or Celiac Disease and if I am right Co-Morbid Pellagra now forgotten for 75+ years since the “War on Pellagra” is now over according to medical professionals’ but sadly the battle rages on for at least the 60 Million American’s alone who get (Infant) Heartburn  once a month not counting the heartburn that grows up to be IBS, UC, Chron’s or Celiac disease.

I always say the number one mistake people make with Niacinamide is not taking it long enough (3 to 6 months 3/day in divided doses) but now I am going to amend that.  The number one mistake people make with GI problems is the mistake of not taking the Vitamin in the first place.  The 2nd biggest mistake is they don’t take it long enough!

But mine has taken The Niacinamide Challenge and his digestive/GI problems are in remission.  And his difficult to help/treat patients who don’t respond to “popular” medicine’s like Linzess etc. are better. And have stopped the medicine because their symptoms are in remission.

They (drugs) are popular because they have drug companies who have the money to advertise them.

I do not.  I do not have a Unique Selling Position (USP).

Nor do Vitamins these days!

You don’t have to take “my Vitamins”.  I don’t sell Vitamins.  But you do have to take the correct one to get better from the correct diagnosis.

No body profits if you get better in 6 months or a year and you no longer need the Vitamin!

I usually end up giving Niacinamide away and often they give it back instead.

Such is the fear of Vitamins these days.

You hear often “you” the average person doesn’t need to take a Vitamin but if you are reading this blog you are not average.  People with Celiac disease and other GI problems are known to be low in a range of Vitamins.

But still people are afraid to take a water soluble Vitamin that is known to help digestion problems.
